HOW TO REPORT A LOST OR STOLEN PASSPORT
The loss or theft of your passport
should be reported immediately to a British Consulate irrespective of whether
you intend to apply for a new passport. You should use form LS01 which you
can hand in at your nearest British Consulate or post to us:

How to apply for a replacement passport
If your passport is lost or stolen in Spain, Portugal
or Andorra and you wish to apply for a new passport you have two options:
If you have immediate travel commitments you should contact your nearest British
Consulate for advice.
If you do not need to travel immediately you should apply for a full British
passport through the Consulate General in Madrid, either in person or by post
at the address shown above.
You will need to submit the following information:
Completed LS01 - Lost & Stolen passport form (available from all Consulates) Completed C1 Passport form, or C2 Passport form Police report, called a denuncia*
2 passport size photographs
*denuncias can be made by phone in Spanish and English, by calling 902 102 112, as well as on the internet in Spanish at the Spanish police's "Denuncias" web page. Otherwise, they can be made at the nearest Comisaria de Policia (In Madrid, this is at Calle General Pardiñas, 90). Some crimes, especially serious crimes involving violence, will usually have to be reported in person). If necessary, and in major incidents, the police will provide an interpreter.
